Deep muscle and SMAS lifts are performed by many surgeons as an alternative to the conventional facelift. They involve going into deeper layers of facial skin, usually below the muscle, and in some cases clear down to the bone. In Dr. Bellin´s opinion, however, the problem with facial aging is one of skin elasticity, not weak muscles. He believes the conventional face lift technique to be far superior and safer.
In a study performed years ago, a conventional face lift operation was done on one side of the face and a muscle-pull operation was done on the other. No one could see any significant difference between the two.
A major problem with the deep muscle operation is that it really only pulls the mid-face, and not the entire face evenly. This often results in a “pulled” effect, especially from the corner of the mouth toward your ear, making for a very unnatural look. In addition, the risk of nerve damage from the deep method and the SMAS method is increased significantly because the surgery is done below the muscle.
The temporal lift
This partial, “compromise” lift improves only the outer brow area, some types of crow’s feet, and the skin and fat on your cheekbones. The incision is kept entirely behind your temple hairline, allowing a limited amount of skin to be removed. The benefits are fewer than those from a full face lift, but swelling and discoloration do not occur, and complications are extremely rare.
The eyebrow lift
This lift is done by making an incision directly above your eyebrow and cutting out some of the forehead tissue to raise your eyebrow up. This procedure leaves a highly noticeable scar, so it is not normally recommended. Since facelift surgery can lift your forehead, it would be a better choice as a way to raise your eyebrows.
Removal of neck fat (double chin)
As you age, you tend to accumulate fat in the upper portion of the neck – what is commonly referred to as a “double chin”. This does away with the youthful contour line between your jaw and neck. A facelift procedure does not always fix this. Dr. Bellin may make an additional incision under your chin to remove the neck fat.
